Sustained reduction of central line-associated bloodstream infections outside the intensive care unit with a multimodal intervention focusing on central line maintenance.

A multimodal intervention focussed on prevention of CLABSI can lead to a sustained reduction of CLABSI rates outside the ICU.

This study focused on preventing central line-associated bloodstream infections (CLABSIs) outside of intensive care units (ICUs). A multimodal intervention was implemented, which included engagement of nursing staff and leadership, nursing education on line care maintenance, competence evaluation, audits of line care, and regular feedback on CLABSI rates. The study consisted of three phases (preintervention, intervention, and postintervention) over a 4.5-year period (2008-2012). The overall CLABSI rate for all participating units dropped from 2.6/1,000 line-days in the preinterventionphase to 2.1/1,000 line-days in the intervention phase and 1.3/1,0000 line-days in the postintervention phase, which is a decrease by 50%. The study suggests that a multipronged approach may be effective in reducing CLABSI rates outside of the ICU.
